Learning Environment Needs Form

The purpose of this form is to provide information about your student as we build classes for the 2024-25 school year. To share information about the educational setting your student needs to maximize his/her learning, fill out our Learning Environment Need form. Please submit this information by April 30, 2024.

Please describe the educational setting your child needs in order to maximize his/her learning. (Please do not ask for a particular teacher.) This might include separation from another student, emotional or academic needs, or other areas of concern. We want to be as intentional as possible when making classroom placements using all the available information. Many factors are taken into account as we strive to create the best learning environment possible for every student at Antelope Canyon Elementary.
